PayPal has failed at designing this form. See, the problem with design is that it’s full of amateurs who think it’s all visual. Design is how something works, how it feels to use, how well it communicates.
In this form, you are supposed to check the items you want to Unsubscribe from, where I would expect to UN-check the items I don’t want. To further complicate things, the sub-labels read like this: “Receive newsletter updates on services”. So am I checking /to receive/ or am I checking the box to unsubscribe? Careful reading of the instructions informs me that I should check the items I do not want, but a better design would make this obvious at a glance.
Follow up:
To make matters worse, I got this confirmation:
We’ve received your request and you’ll be unsubscribed within 10 days.
10 days? Am I unsubscribing via mailed letter? And the confirmation screen does not tell me what I unsubscribed from, giving me less confidence that my intentions were recorded correctly.
Follow up 2:
I guess they’re not concerned with website problems, as their “email contact form” forces me to pick a topic and sub-topic tailored to the kinds of problems they want to respond to, none of which are related to issues with the website.
